Logging in and resetting your password

By answering Yes to the question " Do you wish to complete the Medical and Lifestyle sections* (instead of using the Tele-interview)?", the questions will appear. You must answer them and/or attach the duly signed forms that you will have completed beforehand with your client. *The phone interview is still available and required in some cases.

Access the electronic application at ssq.ca/electronic-application.

To log in, you need to enter your email address and the password provided.

Note: Your username and password will remain the same regardless of the product selected. If you used the electronic application prior to the launch of the simplified or guaranteed issue products, your login information remains unchanged.


If you forgot your password, reset it on the login page. A new password will be sent to you by email. If you don’t see this email in your inbox, check your spam (junk) folder.
