- You have SSQ Privilege - Health Insurance coverage.
WHAT ABOUT EVIDENCE OF INSURABILITY?
- If you apply within 90 days following termination of your group dental insurance, no evidence of insurability is required.
- If you apply after 90 days, or do not have group dental insurance coverage, you will be asked to complete a short dental health questionnaire.
